About Stockton Service Navigation Project
The Stockton Service Navigation Project (SSNP) offers up to four one to one support appointments to help you improve your health and wellbeing by supporting you to access a range of activities. This is known as social prescribing and aims to help people increase their confidence, control, independence, reduce stress and isolation and help people to live healthier lives.
We can help you find an activity such as a walking group, creative arts group, exercise class or social group to support your health and wellbeing. We can also signpost and support you to access other sources of support such as the Citizens Advice Bureau, education providers, counselling, volunteering opportunities and job clubs.

How you access our service
A face-to-face service is available via appointment only Monday to Friday 9am-5pm.
